
HOUSTON, Texas (KTRK) -- A Houston plastic surgeon agreed with the Texas Medical Board to take courses and pay a fine in response to a complaint about two patient outcomes and record-keeping concerns.
Dr. Dare Ajibade met with the representatives from the medical board in June 2026 for an Informal Show Compliance Proceeding and Settlement Conference, according to TMB records.
In an agreed order posted to his profile on TMB's website today, the board describes a patient who died from a pulmonary fat embolism as a result of a surgery performed by Dr. Ajibade that lasted 10 hours.
Another patient who the board says had two "lengthy" procedures within a 48-hour period is also mentioned.
Both patients were poor candidates for their respective procedures due to their medical histories and poor health overall, the agreed order says.
Dr. Ajibade's medical records were described by the TMB as "deficient" in both cases and lacked appropriate consents for the procedures.
13 Investigates is unaware of any lawsuits filed in either case.

According to the board, Dr. Ajibade "reported that he has made changes to his practice so that he performs longer procedures at a hospital facility."
It goes on to say that he neither admitted nor denied the allegations, but that he "appeared contrite and regretful regarding these patient outcomes."
As a result, Dr. Ajibade is required to take a "Medical Recordkeeping Course," complete four hours of continuing education and pay a $4,000 fine.
In December, TMB issued a remedial plan for Dr. Ajibade for another patient. That complaint yielded similar results, including continuing education hours and a fine.
13 Investigates reached out to Dr. Ajibade's attorney regarding the board action but has not heard back.
We also asked the Texas Medical Board for comment and if the penalties were sufficient. We are awaiting a response.
Four other patients have filed medical malpractice suits against the doctor since August of last year.
In May of 2024, Gavin Easley found his mother dead on the bathroom floor of her hotel following a liposuction procedure with Dr. Ajibade in Virginia. His attorney, Peter Anderson, said in a medical malpractice suit that the surgery lasted 6.5 hours, which he called a "highly unusual" duration.
In July of 2024, Rosa Caldera had liposuction with Dr. Ajibade at his clinic in Montrose and was taken by ambulance to the hospital from the clinic. Her lawsuit, filed by attorney Lennie Bollinger, alleges that she suffered third-degree burns and had to have skin graft surgery.
In May of 2025, Ebony Williams underwent a procedure with Dr. Ajibade, according to her lawsuit filed by Ralph Manginello, which led to a hospitalization, "gaping wounds, severe damage and scarring." The suit said she would "require multiple surgeries, multiple weeks incapacitated, limited mobility and disfigurement."
Adebola Britt died in November of 2025, according to her family's attorney Donna Aversano. She said Britt suffered cardiac arrest in the recovery room and died days later in the hospital. The family has not filed suit at this time.
Junis Maldonado underwent liposuction with Dr. Ajibade at a clinic in San Antonio in May of 2026. A lawsuit filed by Beth Janicek on behalf of her family says she suffered multiple seizures and cardiopulmonary arrest during her procedure. Maldonado is expected to suffer "severe permanent cognitive, neurological, and functional disabilities for the remainder of her life."
13 Investigates made numerous attempts to reach Dr. Ajibade regarding those cases. He did not respond, but we received an unsigned email from his clinic, DVA Plastic Surgery, saying they could not comment.
